Under 6 — Date of Birth 08/01/03 - 12/31/04
Under 8 — Date of Birth 08/01/01 - 07/31/03
Under 10 — Date of Birth 08/01/99 - 07/31/01
Under 12 — Date of Birth 08/01/97 - 07/31/99
Under 14 — Date of Birth 08/01/95 - 07/31/97
In order to register, each player will need the following:

Completed Registration and Medical Release forms

Photo copy of your child's birth certificate (if they did not play with our club last year)
- Registration fee payment in the form of cash or check payable to United Soccer Association
Registration fees are $75 for the first player and $70 for each additional player within the same family.It is strongly suggested that parents attend at least one of the Registrations to ensure the correct uniform size is entered for your child on the Registration form. All actual uniform sizes will be available at each Registration.
